Registration Procedures      2012/2013

1.     Verification of Residency:

Verify attendance with one of the following, check one and make a copy.

____Utility Bill (not telephone) Gas, Water, or Electric bill

____Rental Agreement

____Deed to Home/Escrow Papers

____Notarized form Proof of Residence

____Inter/Intra District Permit

____Birth Certificate

____Passport

____Baptism Record

3.     Immunization Card Must have all of the following:

____4 doses of Polio

____5doses of DTP

____2 doses of MMR

____3 doses of Hepatitis B

____2 dose of varicella (chicken pox)

____Mantoux TB skin test (within 1 year)

*If positive please not in red ink on the top right.

____Complete physical exam (Kinder & 1^st only) Signed by Doctor

____Complete Oral Health Assessment (Kinder & 1^st. only) Signed by Dentist

4.     The following enrollment forms must be filled out completely:

____Child and Parent/Guardian Information (front & back)

____Health Information (white form)

____Language Survey (If any of the first three responses on the survey are other than

        English, the student will need to be assessed by the assessment center.

Registration hours are from 9:30 am to 2:00 pm      EXCEPT ON WEDNESDAY from 9:30 am to 12:30 pm

REGISTRATION WILL NOT STARTED UNTIL ALL FORMS HAVE BEEN TURNED INTO THE SCHOOL .
